Please only complete this form at the request of Benny and the Pets AZ, after we've discussed your services.
Inclusive in Boarding:
All dogs in boarding will receive:
A clean, comfortable sleeping area with bedding and appropriate enrichment items for the duration of their stay.
Structured care including feeding, medications, exercise, social time with other dogs, and (if applicable) training.
IMPORTANT: If the pet parent knows or suspects that their above-listed dog(s) is prone to ingesting bedding, toys, or other foreign objects, it is their responsibility to notify Benny and the Pets in advance and request that no such items be provided.
Cancellation Policy:
House sitting and boarding reservations: Must be cancelled at least 3 days in advance
Daycare, dog walks, and drop-in reservations: Must be canceled with at least 24 hours’ notice
Reservations of all kinds cancelled after these allotted windows will receive a 50% refund
No shows without notice: Full fee due
Emergencies: Handled on a case-by-case basis
Payment:
Payment is due in advance of services rendered.
Our always-free payment options are: Cash, check, Cash App, and Zelle.
We also accept payments via PayPal (a 2.89% fee applies if sent as “Professional Services” and Venmo (a 1.9% fee applies if sent as “Professional Services.”
Training Results:
Training is a process that depends on many variables, including consistency, environment, and follow-through from the pet parent. Benny and the Pets does not guarantee specific outcomes or results. Progress will vary between dogs.
Training Termination Policy:
Either party (the pet parent or Benny and the Pets) may choose to end a training program at any time. If the agreement is ended before completion of the scheduled program, a pro-rated refund will be issued for any unused training days or sessions, less any applicable fees for services already rendered.
Punctuality Requirement:
Clients are expected to arrive on time for all scheduled pickups, drop-offs, and private lessons. A late fee of $15 will be charged if the pet parent’s arrival deviates more than 15 minutes from the agreed-upon time. This ensures smooth scheduling and respect for everyone’s time.
Health and Safety:
To receive services, the above-listed dog(s) must be current on core vaccinations (Rabies, Distemper/Parvo) and free of contagious diseases, fleas, and parasites. Preventative flea and tick medications are highly encouraged.
If health issues arise while a pet is in our care, Benny and the Pets will attempt to contact the pet parent. By signing this form, the pet parent authorizes Benny and the Pets to seek emergency veterinary care at the pet parent’s expense.
Transportation Authorization
By signing this form, the pet parent authorizes Benny and the Pets and its representatives to transport the above-listed dog(s) in a motor vehicle for enrichment, training, veterinary care, grooming, or any other essential service.
Grooming
If grooming (e.g., nail trims, bathing, mat removal) is needed during the above-listed dog(s) boarding or housesitting services, Benny and the Pets will arrange services as necessary. The pet parent is responsible for all grooming costs, which will be billed directly or added to the final invoice. An effort will be made to communicate these situations in advance of services being performed.
Media Release
By signing this form, the pet parent authorizes Benny and the Pets to use photos/videos of my dog(s) for educational and promotional content.
Liability
Benny and the Pets will take reasonable precautions to ensure each dog’s safety. However, by signing this form, the pet parent agrees to release Benny and the Pets from liability for injury, illness, escape, or death, except in cases of gross negligence.
By filling out and submitting the following form, the pet parent agrees to all terms and conditions of this agreement.
